Becoming a Patient

What you need to do to be scheduled:

  • Make sure you’ve read the FAQ page.
  • Contact your insurance and verify that Dr. Cathryn Harbor is listed under your specific plan. Check whether or not labs ordered through Quest Diagnostics are covered under your specific plan.
  • Contact our office to check availability for your appointment (540) 463-2882.
  • Submit these forms. You will not be scheduled until you have completed this step. If you have questions or concerns about filling out these forms, contact our office.
  • Provide $250 deposit to secure your time on my schedule. If you do not come to your appointment we will keep this money to cover the cost of our lost time. If you do come, this money will be used to cover your Chart Review.
  • You will need any documents, labs, medical office notes, and medical results from your most recent medical visits. We need these but will schedule you without them. Please call if you have any questions.
  • If you think Lyme Disease is part of your problem, please read the information on this page. If you think mold is part of your problem, please read the information on this page.
  • If you are involved in a lawsuit where your health is relevant, please let our office know before scheduling your appointment.
